Introducing… Connecting Parents to Pathways!
Address the “missing-middle” of parents increasingly presenting at Peach Tree post-Covid seeking moderate-to-severe levels of PMH care.
Is an 8-week, Parent-led, one-on-one program facilitated by a Peach Tree PMH Peer Worker, focusing on a selection of well-being topics using a trauma-informed, strengths-based approach.
Provide an alternative for overwhelmed clinical care services, walking “with” distressed parents along the Continuum of Care toward personal recovery goals while identifying and navigating relevant and appropriate PIMH support services.
Maintain and keep within Peach Tree’s PMH Peer Work “scope of practice” by working collaboratively with PIMH health professionals and organisations to provide distressed parents the best possible care.
Connecting Parents, like the SPP, allows parents a uniquely reassuring soft-entry into PMH care by providing Peer-led, non-clinical services in a safe, reassuring, and validating environment.
How does it work?
The “Connecting Parents” program is unique because it:
- offers a three-stream approach
- focuses on Peer Navigation, which for Peach Tree means “building the village”
- provides Peer-led coaching with a foundation of Lived Experience
- enables Parent-led topic choices (three out of eight domains)
Process & domains:
Intake interview (phone) / sessions in-person or online
Week 1: Getting to know you / Wellbeing Domains
Weeks 2+3: Parent chooses 3 of 8 Wellbeing Domains, begins 1st domain in the CP Workbook with Peer Worker
Weeks 4+5: As above with 2nd chose domain topic
Week 6+7: As above with 3rd chosen domain topic
Week 8: Parent and Peer Worker reflect together and plan for immediate, medium, and long-term goals
Resides in Metro North or Metro South Brisbane area
Experiences PMH distress resulting in reduced psychological and functional capacity
